GPA 3.99 MCAT 522, List too top heavy?

Can't believe this cycle is already here! I'm feeling good about my stats but I am worried that I won't stand out to Top 20 schools among students of the same caliber. I have 17 schools on my list right now, and 4 I'm not certain about but considering. Based on my stats and extra-curriculars/research/shadowing, am I being too optimistic? Should I add more mid-tier schools? I'd appreciate any feedback or advice!
